The Accucare Home Health Services home healthcare team members will work individually with the patient to facilitate strength and recovery:

• Nurses will monitor vital signs, medications, conduct thorough physical examination and review of body systems, educate in disease management, as well as perform lab draws as needed.
• Physical Therapists will promote healing and strengthening of motor functions, prevent complications, and promote maximum rehabilitation potential by giving appropriate therapeutic exercises and patient education.
• Occupational Therapists will assist with re-training for self-care skills including feeding, dressing, bathing, grooming, personal hygiene, toileting and other daily living activities.
• Speech Therapists will help with voice disorders, improve receptive and expressive speech and language skills, as well as evaluate and treat swallowing and hearing disorders.
• Medical Social Workers will help the patient and family work through various social, psychological, cultural and medical issues resulting from an illness. Our medical social workers will also help with long term planning and provide education regarding available community resources, to include assistance with Advance Directives.
• Home Health Aides are available as needed to assist with personal hygiene with the goal of promoting independence and healing.
