  • Accucare Home Healthcare provides Therapeutic options for rehabilitation in patients who needs to regain strength and functioning after a medical diagnosis, an injury, or a surgical procedure. Each of our therapists provide evaluations, treatment and education for patients with post-surgical procedures and new and ongoing diagnosis that can be treated with real obtainable goals.

    Physical Therapy (PT) services help patients restore and regain strength and movement after loss of function due to aging, an illness, a hospital or facility stay, or surgery. Physical Therapist evaluates pain, physical and mental health, mobility, range of motion, gait, and treatment plan.

    Occupational Therapy (OT) services help patients with conditions such as COPD, depression, stroke, Alzheimer’s disease, and arthritis or generalized weakness. Occupational Therapists recommends and instructs to use of adaptive equipment such as walkers, wheelchairs, orthoses and devices to help with daily living activities.
    Our Occupational Therapists evaluates the patients home, the patient’s ability to perform activities of daily living (ADL), any limitation to independence, and develop a care plan to restore decreased function.

  • Medical Social Workers helps patients and their caregivers with social and emotional difficulties and evaluates each patient’s situation to determine limitation, barriers, or safety issues that may arise. Social Workers provides knowledge and information about community resources such as meal delivery and preparation, finance, transportation, and long term care planning.

  • Home Health Aide’s provides hands on assistance to individuals who are unable to remain independent (with a family member/Caregiver) in their own home while receiving treatment from a Clinician. All Home Health Aides are under the supervision of a Registered Nurse and responsibilities may vary from patient to patient which include short term assistance in daily living.

    Our Home Health Aids focus on:

    I t’s a 3-4-week program, patients with complex care needs to receive specific tools, specific skilled nursing care protocols and are supported by the transition care team. The transition program comprises of logistical arrangement, coordination with health professionals, and administration of medications and treatment, provide rehabilitative services and instruct patient/ caregivers the self-management skills.
